Dofetilide(CAT: A000803) is a class III antiarrhythmic agent used primarily for the treatment of atrial fibrillation and atrial flutter. It works by selectively inhibiting the rapid component of the delayed rectifier potassium current (IKr), thereby prolonging the action potential duration and refractory period without affecting conduction velocity. This targeted mechanism helps restore and maintain normal sinus rhythm. Dofetilide is administered orally and requires careful dose titration and electrocardiographic monitoring due to its risk of QT interval prolongation and torsades de pointes. It is particularly suited for patients with structural heart disease who require rhythm control and have limited alternatives.
